Returning Student Registration SY 2011-12

(Any student who is currently enrolled at Puckett’s Mill Elementary School)

STEP 1: Complete this required online registration form for each returning student.
The last day to complete the form will be August 2nd. The information entered is secure. Required school forms will be generated from the information given. (It may take a minute to load.)

STEP 2: If you would like information about our school nutrition program, please click here.  Returning families currently receiving free and reduced lunch or those interested in applying for free and reduced lunch must complete applications annually, Click here for online application.

STEP 3: If you have not created a GCPS Parent Portal account, download and complete the Parent Portal form. For more information on Parent Portal, click here.
Note:  You must bring the new form into the main office along with Driver's License for verification.

STEP 4: Bring the following documents into the main office, if applicable:

Immunizations:  If your child's immunization record has expired, you will need an updated Immunization Form from your child's doctor or the Health Department. Click here for more details.

Residency Affidavits: Residency Affidavits are only valid for one school year. If the Residency Affidavit we have on record has expired, please click here to print a new one, complete it, and have it notarized. Please return it to school with a current utility bill (gas, water or electric).

 Transportation: If afternoon transportation is to an Alternate Address on a GCPS bus other than home, download and complete the Alternate Transportation form and bring it to the main office for approval by the GCPS Transportation Department.
